What was the installation/upgrade media you used? Was the same media used with both hosts?
If it was the same media used both times it sounds like it could be corrupted.
I tried a test installation of Windows 8.1 Pro 64-bit using an ISO file and it completed without errors on the current version of Workstation 12.1.1.
